How to Apply

To apply for the Pediatric Radiology Fellowship, please submit the items below (electronic submission is fine to courtney-bork@uiowa.edu):

  1. Completed application that can be e-mailed, mailed or faxed
  2. Curriculum vitae
  3. Statement of purpose: Summary of your training background, areas of particular strength and interest, and a brief description of how you would hope to utilize and apply your fellowship training
  4. copies of all USMLE exam reports and ECFMG certificate (if applicable)
  5. three letters of recommendation - letters should come from those persons familiar with your recent training and current professional activities. Please instruct reference individuals to mail their letters of recommendation directly to Courtney Bork (courtney-bork@uiowa.edu), at the address listed below
  6. Copy of your ECFMG certificate. For non-residents: We sponsor J-1 and H1-B visas, please mention which you plan to pursue and if you are requesting a 4-year ABR alternate pathway. After your application is complete, it will be reviewed and a decision made regarding an invitation to interview. 

Criteria for Candidacy

All applicants must have completed a Radiology Residency program prior to submitting an application, as well as their USMLE steps 1, 2 and 3. Ability to obtain a resident medical license in the State of Iowa: https://dial.iowa.gov/licenses/medical/physician#eligibility. Candidates are preferably in the process of preparing for certification in the American Board of Radiology (ABR).
